ChiroEnroll ("we," "us," or "our") operates chiroenroll.com and provides chiropractic insurance credentialing services. This Privacy Policy explains how we collect, use, protect, and share information you provide to us.

Information We Collect

To provide credentialing services, we collect the following information:

• Personal identifying information: name, date of birth, Social Security Number (last 4 digits for some applications), contact information
• Professional credentials: NPI number, chiropractic license number and state, DEA number, CAQH ID
• Practice information: practice name, address, Tax ID (EIN), phone number
• Financial information: bank routing and account numbers (for EFT setup with insurance payers only)
• Insurance information: malpractice carrier, policy number, coverage dates
• Documents: chiropractic license, malpractice certificate, NPI confirmation, voided check, W-9, and other credentialing documents
• Payment information: processed securely through Stripe — we never store credit card numbers
• Email address: used for service communications and, if you opted in, our checklist newsletter

How We Use Your Information

Your information is used exclusively to:

• Complete your insurance credentialing and payer enrollment applications
• Set up and maintain your CAQH ProView profile
• Communicate with insurance companies and payers on your behalf
• Send you status updates about your credentialing applications
• Provide access to your real-time credentialing dashboard
• Send service-related communications (never marketing without consent)

We do not sell, rent, or share your personal information with third parties for marketing purposes under any circumstances.

HIPAA Compliance

We handle Protected Health Information (PHI) in compliance with the Health Insurance Portability and Accountability Act (HIPAA). As a Business Associate under HIPAA, we implement appropriate administrative, physical, and technical safeguards to protect the confidentiality, integrity, and availability of PHI. We will sign a Business Associate Agreement (BAA) upon request.

Data Security

• All data is transmitted using 256-bit SSL/TLS encryption
• Documents are stored using Uploadcare's encrypted cloud storage
• Form submissions are transmitted securely via Formspree with encryption in transit and at rest
• Access to your data is restricted to credentialing specialists assigned to your account
• We do not store banking information beyond what is required for EFT setup

Third-Party Services

We use the following third-party services to operate ChiroEnroll:

• Stripe — payment processing. Stripe's privacy policy applies to payment data: stripe.com/privacy
• Uploadcare — document file storage. Uploadcare's privacy policy: uploadcare.com/about/privacy-policy
• Formspree — form submission processing: formspree.io/privacy
• Netlify — website hosting: netlify.com/privacy

We do not share your personal or professional information with these services beyond what is technically necessary to operate our service.

Data Retention

We retain your credentialing information for as long as you are an active client, plus a reasonable period thereafter for record-keeping purposes. You may request deletion of your data at any time by contacting us at support@chiroenroll.com. We will comply within 30 days, subject to any legal retention requirements.

You have the right to:

• Access the personal information we hold about you
• Correct inaccurate information
• Request deletion of your data
• Withdraw consent for communications
• Request a copy of your data in a portable format
